Youth volunteers must submit a Youth Code of Conduct form signed by a parent or guardian. These can be completed and emailed to [email protected] or brought in-person to the Office of Faith Formation at Immaculate Conception Church. This form is valid for the remainder of the 2024-2025 academic year.
